A cube pessary is typically used in cases of advanced pelvic organ prolapse when other pessaries are ineffective or not retained. It’s one of the most supportive types due to its suction-based design.
When to Use a Cube Pessary:
Severe (Stage III–IV) uterine or vaginal vault prolapse
Especially when the vaginal walls are too weak or wide to hold a ring or donut pessary.
Prolapse with poor vaginal tone or laxity
The cube creates suction against the vaginal walls, making it ideal for patients with significant tissue laxity or loss of pelvic floor support.
When other pessaries are expelled
Because of its suction, a cube is often used when ring, Gellhorn, or donut pessaries don’t stay in place.
Temporary or short-term use
Often used during the day and removed at night, due to potential tissue pressure if worn continuously.
Non-surgical candidates or those awaiting surgery
Provides immediate support and symptom relief without invasive procedures.
Special Considerations:
Requires daily removal and cleaning to avoid pressure ulcers or vaginal irritation.
Not ideal for patients who cannot manage self-care unless a caregiver or clinician is involved.
