A donut pessary is typically used for moderate to severe pelvic organ prolapse, especially when other pessary types fail to stay in place or provide adequate support.
When to Use a Donut Pessary:
Moderate to Severe Uterine or Vaginal Vault Prolapse (Stage II–IV)
The donut’s large, rounded shape provides significant support for prolapsed organs.
Cystocele and Rectocele (Anterior and Posterior Vaginal Wall Prolapse)
Offers circumferential support for multiple-compartment prolapses.
Patients with Poor Vaginal Tone
Ideal when the introitus is too lax for smaller pessaries like rings or Shaatz to stay in place.
When Other Pessaries Are Expelled
Donut pessaries are often retained better due to their bulk and shape.
Non-Surgical Candidates
Often used in elderly or medically complex patients who are not suitable for surgical correction.
