A long stem Gellhorn pessary is typically used in the management of advanced pelvic organ prolapse, particularly in the following situations:
When to Use a Long Stem Gellhorn Pessary:
Severe or Stage III–IV uterine or vaginal vault prolapse
Especially when the vaginal walls are too lax to support other types of pessaries.
Post-hysterectomy vaginal vault prolapse
The long stem helps anchor the pessary in place when the vaginal apex lacks natural support.
Incompetent vaginal introitus
When the vaginal opening is too wide or weak to retain a ring or cube pessary.
When other pessaries fail or are expelled
The Gellhorn’s disk-and-stem design offers better retention in cases where other types fall out.
Non-surgical candidates
For patients who are not good candidates for surgery due to age, comorbidities, or personal preference
The long stem variation is particularly helpful in patients with deeper vaginal canals or greater prolapse descent, where a standard Gellhorn may not stay in position.
